Part Time Human Resource Director

We are looking for a Human Resource Associate to join our team. You will serve as the primary point of contact for our Team members and address questions about all areas of human resources including but not limited to benefits, payroll, HR policies, staffing and employee engagement.

You will support all members of the leadership Team by helping our leaders fill their staffing needs, on-board new members, and work collaboratively with NDHRC’s Administrator in Center-wide initiatives.

We will use your HR knowledge, skills, and abilities to maintain all HR systems and compliance activities with timely oversight and updates driven by changes in state and federal laws. You will develop and maintain all relevant documents in employee files to ensure our compliance and CMS survey readiness.

We are open to your qualifications. A Bachelor’s degree in Human Resources or a Bachelor’s degree in Business with a concentration in Human Resources is preferred. Relevant Human Resource experience and tenure in Long Term Care (SNF) industry, without a degree will also be considered.

While experience is preferred (2 to 3 year minimum), we welcome a recent college graduate and will consider any experience you may have gained in an external internship.

Director of Therapeutic Recreation

After 35 years our current Director of Therapeutic Recreation is going to enjoy her retirement and we are looking for a Director who we hope will join us and make a life time career at NDHRC. As the leader in Therapeutic Recreation you will plan individual and group programs in accordance with patient/residents’ needs, preferences, interests, abilities, and consistent with treatment goals and interventions. Train and provide supervision to recreation staff and volunteers Maintain required documentation; participate in budget planning Develop positive relationships with patient/resident’s family and the community. Use community resources to create or enhance recreation programs. Your skills, enthusiasm and passion will improve the lives of our residents.

Bachelor degree in therapeutic recreation preferred or completion of NAAP/NCCAP Basic and Advanced Management Course for Activity Professionals Certification in accordance with regulatory agencies governing the center, by the National Certification Council of Activity Professionals (ADC) or the National Council of Therapeutic Recreation Certification (CTRS) Two years’ experience in a social or recreational program within the last 5 years, health care setting preferred Equal Opportunity Employer Minorities/Women/Protected Veterans/Disabled

 


Part Time Director of Admissions

Notre Dame Health and Rehabilitation Center (NDHRC) offers you an opportunity to be a member of a community and team who support each other and provides meaningful work impacting the lives of others. The Director of Admissions will develop and implement referral and admissions plans to create referral flow, admissions processes including community based sales, manage new referrals from admission commitment to post admission status and placement in the Center. You will build relationships with area Assisted Living, Senior Living Communities and other like communities to support census development and customer relations. You will work with operational team members to ensure customer satisfaction and your experience and use your skills with Medicare/Medicaid regulations and Ascend process. Reporting to the Center Administrator and work to support strategic Quality Mix (Qmix) and Average Daily Census (ADC) goals.

Minimum of three (3) years’ experience in a healthcare setting. Minimum of two (2) years college education in the field of health care administration, business management, and/or marketing or public relations preferred. Ability to adjust work schedule based on centers’ business need, specifically incoming admissions.

 


Part Time Director of Social Work

As our primary social worker supporting our residents and families you will:

  • Complete clinical assessments and social service documentation as required by state / federal mandates and facility policy
  • Interviews residents and their family members to solicit information regarding the resident’s strengths, weaknesses, coping strategies and lifestyle prior to placement and develops and initial psychosocial assessment along with a detailed social history within the clinical record
  • Develops and maintain resident psychosocial assessments and quarterly/routine care plans, • Educates residents, family members, and / or significant others regarding advance care planning and community resources
  • Works with the interdisciplinary team to assure resident and family concerns are addressed in a timely manner. Advocates for the needs of residents in ensuring resident rights,
  • Educate residents, families, and staff on resident rights, resident abuse, mistreatment and neglect regulations to ensure residents are protected,
  • Provides information and documentation to residents and families with regards to access to Medicare, and financial and insurance benefits.
  • Collaborates with social service team members to ensure the needs of residents are met with professionalism, promote resident dignity, and resident choice.

Requirements Licensed Social Worker (LSW or LCSW) required. Degree in social work or related field. Minimum two years experience working in the human services and/or geriatric services field required. (Long term care setting preferred) Must have strong clinical skills and demonstrate the ability to provide a safe and emotional support to residents of the facility.
