Notre Dame Health and Rehabilitation Center is equipped to provide the highest quality care to patients who are referred from area hospitals after a hospitalization.

• Diagnostic Testing Services – Blood work, X-rays and EKGs are coordinated by our Nursing Department and completed on-site.
• IV Therapy
• Wound Care
• Bladder Care
• In-House Podiatry, Dental, Audiology, and Ophthalmology Services

Notre Dame Health and Rehabilitation Center provides in-house podiatry, dental, audiology, and ophthalmology services through Health Drive. Residents are visited monthly to address their service needs.

Referring Hospitals 

Notre Dame Health and Rehabilitation Center accepts referrals from all local hospitals, including: Norwalk Hospital, Stamford Hospital, Bridgeport Hospital, St. Vincent’s Medical Center, Danbury Hospital, Griffin Hospital, Gaylord Hospital, Yale-New Haven Hospital and Milford Hospital. All referrals from the community are reviewed and screened by our Medical Director.
